An EMR is used to record patient health information in a digital format that allows the provider(s) to track a patient’s health over time, improving the quality of care. EHR refers to the shared information from all clinicians involved in a patient’s care.

Demographic information is crucial for patient identification, efficient practice management, and accurate billing. In addition, this information can be used to provide a look at your practice’s performance results in terms of demographic groups such as race, sex, and age.

When a new patient calls to schedule an appointment, you will need to begin the patient registration. This
means that you will need to collect some preliminary patient data in order to start a patient record. To
obtain complete demographic information, you may have the patient complete a registration form on the day of their appointment or you may request the patient complete this information via a secure website
before his or her appointment. Some clinics will mail extensive registration forms or special questionnaires to be brought in on the appointment date.

In the front office, you are responsible for scheduling patient appointments. In order to schedule appointments, a matrix must first be established. This matrix shows office hours and available patient hours for each provider. Office hours are the time when the ongoing responsibilities of office management are
completed while patient hours are those times during the workday that are dedicated to patient visits in the clinical back office.

In preparation for patient appointments, you must be sure that relevant patient information is organized and ready for the health care team. This includes verifying receipt of any related test results and documentation received from outside the clinic, as well as ensuring the day’s schedule is easily accessible to any staff who need it

As patients arrive to the clinic, the administrative medical assistant will perform the check-in tasks. These tasks include updating the patient’s status with your scheduling system; having the patient complete or update registration and acknowledgment forms; scanning insurance cards; running online eligibility reports as necessary.

Co-payments, deductibles, and coinsurance are all part of the everyday dialect of medical practices. Insurance companies negotiate pricing for their insurance plans with the understanding that copayments are part of the contracted rate. Failure to collect that fee comes right out of the practices or provider’s pocket. During the patient check-in, when verifying benefits, you will ask whether the member’s plan has a co-payment, coinsurance, or deductible.

Clinical documentation is at the center of every patient encounter in the back office. In your role in the medical office, you may be one of the primary caregivers obtaining patient data and entering clinical documentation in the patient’s medical record. Clinical documentation includes any information regarding the patient’s health status and the care delivered to the patient.

Your interactions with and care for patients extend beyond a single office visit. A patient’s visit will often result in many follow-up tasks to be completed. In addition to performing these tasks, you will also be responsible for updating the patient’s record to show the completion of these orders and tasks.

Ordering new or renewing existing medication prescriptions has become quite simplified using the EMR. E-prescriptions are computer-generated prescriptions created by the health care provider and sent directly to a pharmacy. Instead of the provider writing out the patient’s prescription on a piece of paper, it can be entered into the computer and sent electronically to a pharmacy of the patient’s choice. This electronic transmission reduces the amount of time pharmacists and pharmacy technicians need to interpret handwriting, follow up with the provider for clarification, and enter information manually.

All medical procedures and health care services provided to patients (including medical, surgical, and diagnostic services; medical supplies; durable medical goods; and non-physician services) have unique codes used for billing purposes. Each of these procedure codes used in your clinic will have a fee associated with it.

After a patient visit, health care providers submit and follow up on claims with health insurance companies in order to receive payment for services provided. This is known as medical billing. Most insurance companies use the same process for submitting claims. The health care provider applies diagnostic and procedural codes to the services and procedures provided to the patient. Each procedure code has a predetermined fee. HIPAA guidelines require practices to send their billing claims electronically for services involving patient interactions.

Just like any other business, medical practices rely on cash flow coming into their practice to sustain their existence. Medical services are provided for an established fee, and patients may pay that fee in a variety of ways. Some patients are cash-only patients, meaning services provided to them are expected to be paid for in full at the time of service unless other payment arrangements are made.
